As someone who is creating a "system" from a mash-up of D&D from old editions and OSR inspired products this is a good list to check off from.
I would add off the top of my head:
What classes can use what specific magic items? If you are pulling from various sources and adding new classes, etc., it will be good to know this beforehand.
Here's another which comes up all the time yet I rarely see addressed:
My ally is incapacitated (unconscious, dead, paralysed, etc). Can I drag them to safety? How fast can I move in so doing? Does it just use my movement or my action as well? Or if not my action perhaps it just occupies both hands? What if two or more of us are helping?
Exactly how many wheels of cheese does one Rat King eat at a time, or, over one period of time?!? Or is he throwing a big cheese wheel fete for all his rat subjects, and they eat all the wheels of cheese in one fantastic, gluttonous evening? How many rat subjects show up to the party? Are there any non-rattus or even non-muridae dignitaries invited?! And if so, do any of them suffer from lactose intolerance??? Also, where's the encounter matrix that pairs wine served by type of cheese wheel?!?!? So many questions I'm gonna need this TTRPG to answer... 😀
* A PC is surrounded by monsters. How many of them can attack in a round? What if the PC is holding a 5ft or 10ft corridor from a hoard of monsters, how many can attack then?
* What happens if a side decides to retreat or flee from combat?
* When is something so easy that a PC doesn't have to roll for it? If your game uses Difficulty Ratings or similar: How should the GM decide which difficulty rating to use?
* What happens when you fail at climbing, stealth, pick-pocketing and similar?
